Entry forms are available at Local Style, Big Island Surf (Hilo), Miller Surf or Pacific Vibrations, or you can easily download it here.

**Paid entries must be received by the Wednesday before each event to avoid beach entry fees.**

You will need ALL 3 FORMS to enter! A parent must sign if under age 18.

Mail it with your paid entry to us at HASA-Hawaii Chapter, po box 685, Hilo, HI 96721. Please do not send it to the Florida address on the form.
