To surf in HASA events for the 2009-2010 season, each person must mail in TWO forms by the deadline: HASA Entry Form and HASA Member Contract and Waiver. The Contract/Waiver must be mailed in with your first entry, by the deadline, or you will not be entered in the event. All entries must be mailed and RECEIVED BY the entry deadlines. We will not accept entries at the beach, at work, at home, or anywhere but the PO BOX. Late entries will be returned. We recommend mailing your entries at least 5 days before the deadlines.
A limited number of entry forms may be available at Local Style (Hilo), Orchidland Surf (Hilo), or Pacific Vibrations (Kona), or you can easily download and print it here.
BEACH ENTRIES WILL NOT BE ACCEPTED.

ALL MEMBERS MUST SIGN THIS FORM, REGARDLESS OF AGE. MEMBERS UNDER 18 MUST ALSO HAVE A PARENT OR LEGAL GUARDIAN SIGN IN ADDITION TO THEIR OWN SIGNATURE.
